?  Are Care Coordination & Concierge Care services covered by Medicare or Insurance  ?


Medicare does not cover Care Coordination. Long Term Care Insurance may cover some home  care services and some aspects of Care Coordination. Consult your insurance company or  review  your plan to find out the details of coverage. Concierge services are “fee for service” and are therefore considered an out-of-pocket expense.


?  What geographic areas do you cover  ?


We provide services in the greater Columbia area, including Irmo, Forest Acres, and

 Northeast Columbia.


?  What is a Care Manager  ?


A Care Manager Certified is a professional, such as a social worker, nurse or a human service professional certified by the National Academy of Certified Care Managers. Care Managers help families with immediate or long-term care needs. To find out more information visit www.naccm.net. This includes an in-person assessment, the development and review of a care plan and arranging and monitoring care services. Care Managers do not provide hands-on-care however; provide caregiver consulting and coaching services.  


?  How Can a Care Manager Help Me  ?


A Care Manager may….

 

Conduct a thorough assessment to identify problems and provide solutions. We evaluate the  person’s physical, social and ADL needs.

Make recommendations for care options and available services

Review medical issues, living options, care needs and offer referrals to specialist as needed

Screen, arrange and monitor in home help or other community services

Act as liaison for families at a distance, overseeing care and quickly alerting families to  problems

Assist with moving an older person to or from home,  a retirement complex, assisted living  or nursing home

?  How do you bill for your services ?

Care Coordination is billed at a rate of $35.00 per hour with a two hour minimum.  Concierge  Care services are billed at a rate of $30.00 per hour per staff person with a one hour  minimum. Clients who choose to purchase a monthly membership package receive a $5.00 discounted hourly rate.  

Concierge services are billed from the time we leave our office and ends when we return to  the office.

A mileage fee of $.55 is charged for any travel after a 30 mile radius. We bill our clients “a  la carte” or monthly according to our client’s preference.

We accept cash, credit and debit cards. You may also set up an expense account with us.  You can put as much money as you like into your account each month.

A deposit equivalent to one hour of selected service is charged up front.  Discounts can be  used for membership packages.

All expenses and fees are due at the time of service. The client incurs all additional  expenses (groceries, medication co-pays, personal items, etc.).


?  What is your cancellation policy  ?

We ask that you please schedule services “at least” 24 hours in advance.  

A 12 hour notice is required for cancellation of scheduled services. Less than a 12 hour  notice will incur a fee of 50% of your deposit.
